What is After Hospital Care?

After Hospital Care is a type of dedicated support provided in the comfort of the home, helping you or your loved one to recover safely and independently following a hospital discharge.

Whether you are recovering from planned or emergency surgery, a fall or an illness, coming out of hospital can be a challenging time. It can be an adjustment to go from receiving round the clock care from medical staff to living independently at home again. This can be especially true if you are feeling weaker or more vulnerable than before you went into hospital.

Our After Hospital Care focuses on supporting this adjustment so that it’s as safe and stress free as possible. With nearly 33% of patients feeling unprepared at discharge, our Care Assistants are here to support you or your loved one with whatever is needed to ensure a comfortable recovery.1 This could include assisting with hygiene and providing medication reminders to making meals and completing household tasks.

How much does it cost?

Our Care Assistants can be on hand from the moment you arrive home, helping with daily tasks, personal hygiene and mobility support at a pace that suits you. We work closely with you and your loved ones to ensure a seamless transition, so you can focus on what matters most — getting back to yourself, in the comfort of your home environment.

Medication support

Coming home after a hospital stay often means adapting to new medications or changed medication routine, which can be a lot to manage alongside the demands of recovery. Having reliable support in place from the outset can make this transition smoother and less stressful for you or your loved one.

Our Care Workers are there to help ensure nothing gets overlooked — providing timely medication reminders, collecting prescriptions from the pharmacy and keeping family members informed where needed.

People recovering after surgery

According to Age UK, one in six patients over the age of 75 is readmitted to hospital within 30 days of being discharged.2 For many people, the period immediately after returning home is one of the most physically and emotionally demanding parts of the recovery process. How does Alina Homecare compare to an agency?

Agencies are not regulated and quality cannot be guaranteed. We provide fully regulated services that are inspected by the Care Quality Commission (CQC). We also provide full and exclusive training for our Care Workers, whereas private and agency Carers may not have comprehensive training. In addition to this, our services are also backed up with central support; this means you can count on us even if your Care Worker needs time off, as our local Team can step in to ensure continuity of care.

How is After Hospital Care monitored?

Like all our services, our After Hospital Home Care is regulated by the CQC. The CQC is an independent regulator that inspects our services to ensure our quality standards are maintained. This includes checking that our care is focused on our clients as individuals, as well as that we keep you safe at all times. In addition to this, we are proud to partner with Birdie. Birdie’s innovative care technology enables us to digitally log and monitor the support we provide.
